Our physicians welcome patients for routine eye exams.  At this visit, our experienced team of ophthalmic assistants and physicians will work together to check your vision, determine whether your vision could be improved with glasses or contact lenses and examine your eyes carefully with specialized technology for the presence of any eye conditions or diseases that might threaten your eyesight.  Many people do not realize that some conditions like glaucoma and retinal disease may create few or no symptoms, especially in the early stages of the disease.  Early detection of conditions like these allows for more effective treatment and a better prognosis.

Please bring the following forms with you to your appointment:

  4. Authorization for Release of Information to Family Members
  5. Medical History Questionnaire
  6. Refraction
  7. Medical Records Release Form

Also, in order for us to provide you with the very best care, it is very important that you bring the following items to your visit:

  • A list of your current medications
  • Bring your glasses/contact lens to your visit
  • Insurance information- Medical Insurance card and any Vision Care information.
  • Your Current Driver’s License
  • Information about what treatments and medications have been tried in the past
  • Medical records from prior visits that you think may be helpful
