3. Patient Financial Policy

Also, in order for us to provide you with the very best care, it is very important that you bring the following items to your visit:

• A list of your current medications
• Bring your glasses/contact lens to your visit
• Insurance information- Medical Insurance card and any Vision Care information.
• Your Current Driver’s License
• Information about what treatments and medications have been tried in the past
• Medical records from prior visits that you think may be helpful
